Routinely collected electronic health records (EHR) can provide a rich data source for investigators, but these data present unique inferential challenges. This presentation explores major challenges in seeking causal inferences via EHR data, using the National Clinical Cohort Collaborative (N3C) database and the outcome of Long COVID as a recurring example. We will discuss common pitfalls in analyses of EHR data, such as heterogeneous monitoring and high dimensionality, and methodological approaches to address these challenges. I will also describe several applied studies that evaluated the impact of preventive interventions, such as vaccination or medication repurposing, on Long COVID risk.
